Diagnostic error, defined as wrong, missed or delayed medical diagnosis, occurs in 10 to 15% of diagnostic encounters, is the largest contributor to medical malpractice claims, and is responsible for 40,000 - 80,000 deaths per year in the United States.
Vaccines usually consist of inactivated viruses that prompt the immune system to remember the invader and launch a strong defense if it later encounters the real thing. However, this approach can be too risky with certain viruses, including HIV.
