Scientists have identified some of the first genetic triggers for cancer as mutations in specific oncogenes or tumor suppressor genes. Full-blown tumors and metastatic cancers, however, often exhibit many genetic mutations, sometimes dozens in a given tumor. An important scientific question, and one with significant clinical implications, has been what happens after the initial mutation that leads to dangerous later-stage cancers with multiple damaged genes.
Research has demonstrated that the risk for developing coronary heart disease depends on a host of risk factors that are related both to lifestyle and genetics. In a new study from Brigham and Women's Hospital, Washington University School of Medicine in St. Louis, and Massachusetts General Hospital, researchers tested whether a composite of genetic variants could identify the risk of cardiovascular death and heart attacks as well as identify individuals who derived greater clinical benefit from statin therapy.
The death rates of individual heart surgeons have been published in England and several US states for some years. A high profile investigation into high death rates at a certain cardiac centre motivated this in England. For other surgery types, death rates have been reported only at the hospital or trust level. NHS England's "Everyone Counts" policy, introduced this Summer, means that outcomes of ten types of surgery will be reported at the individual surgeon level.
The measure, which would keep the federal government functioning until March 18, funds most programs at their current level. It does, however, trim $4 billion by targeting certain programs — but the health law is not "de-funded" in this plan. Still, governors are uneasy about the impact likely federal spending reductions will have on their budgets.
