The 2012 South African TB Conference opened Tuesday night in Durban, with the Treatment Action Campaign, Section27, and Oxfam delivering "a memorandum containing five demands to conference organizers shortly before the opening," health-e News Service reports.

Overall, 306 facilities reported 94 preventable medical errors in 2009, according to a report released Monday by the Indiana State Department of Health. That's down from 105 errors reported each year in 2008 and 2007.
