After a tumultuous week of party infighting and leadership stumbles, congressional Republicans are focused on calming their divided ranks in the months ahead, mostly by touting proposals that have wide backing within the GOP and shelving any big-ticket legislation for the rest of the year. Comprehensive immigration reform, tax reform, tweaks to the federal health care law - bipartisan deals on each are probably dead in the water for the rest of this Congress.
The Associated Press reports on "the national surge in elderly inmates whose medical expenses are straining cash-strapped states and have officials looking for solutions, including early release, some possibly to nursing homes. ... That burden is becoming greater as the American Civil Liberties Union estimates that elderly prisoners — the fastest growing segment of the prison population, largely because of tough sentencing laws — are three times more expensive to incarcerate than younger inmates.
Writing in the BMJ Blog, George Alleyne, director emeritus of the Pan American Health Organization (PAHO), reflects on global health challenges and opportunities, writing, "There have been massive changes not only in the basic health problems, but also in how we have addressed them."
Although sedatives are often administered before surgery, a randomized trial finds that among patients undergoing elective surgery under general anesthesia, receiving the sedative lorazepam before surgery, compared with placebo or no premedication, did not improve the self-reported patient experience the day after surgery, but was associated with longer time till removal off a breathing tube (extubation) and a lower rate of early cognitive recovery, according to a study in the March 3 issue of JAMA.
Ahead of next week's replenishment meeting of the Global Fund to Fight AIDS, Tuberculosis and Malaria in New York, IRIN/PlusNews examines the challenges associated with trying to ramp up programs worldwide to meet global health targets.
